Keith Alan Forman

Born on October 19,1981 in Baton Rouge, passed away to be with the Lord on Tuesday, April 12, 2022, at his residence in Albany, LA. He is survived by his children, Logan Scott Smith, Autumn Lee Forman, and Dawson Lane Miller, his parents Charles E Forman II (Chuck), Angela F Sasso and (George J Sasso). Siblings: Jeremiah S Sasso, Angelina S Ferguson. His Aunt Cindy Forman, Uncle Brian Vicknair, Uncle Jason P Dixon, and Grand Mother Velda H. Dixon. He worked as a carpenter for most of adult life. He was loved and will be missed. He is preceded in death by his significant other Lakin Miller and Grandparents Charles E Forman Sr. and Patricia L Forman, Dallas L Dixon. His Aunt Phoebe Vicknair.
